Rethinking cardiometabolic care

The Challenge

Cardiometabolic diseases — including hypertension, diabetes, obesity and cardiovascular conditions — represent a major and growing burden for healthcare systems across Europe.

Patients living with cardiometabolic diseases frequently experience multiple interconnected chronic conditions requiring coordinated care across specialties and healthcare settings.

  • Fragmented, with limited coordination across disciplines and care settings.
  • Insufficiently personalised, relying on standardised approaches rather than patient-specific needs.
  • Reactive, focusing on acute events instead of prevention and early intervention.
  • Digitally disconnected, with isolated tools and limited integration of real-world data.

A new model for connected care

IMPACT-MED proposes a paradigm shift towards a more integrated and proactive model of care. At its core is the Multidisciplinary Cardio-Metabolic Care (MCMC) framework, designed to:

  • Integrate multiple medical specialties into coordinated care pathways.
  • Connect clinical and patient-generated data across settings.
  • Enable personalised and continuous care.
  • Support patients throughout the full disease trajectory.

 

This model is supported by an advanced digital ecosystem combining:

Clinical use cases

IMPACT-MED will validate its multidisciplinary cardio-metabolic care (MCMC) framework through four real-world clinical studies designed to improve prevention, monitoring, personalised treatment and long-term disease management across the cardiometabolic continuum.

AI-driven hypertension management (UC1)

Improving blood pressure control and enabling earlier detection of cardiovascular complications in high-risk patients through AI-based risk stratification, remote monitoring and digital follow-up tools.

Remote monitoring for cardiovascular (UC2)

Strengthening outpatient follow-up and continuity of care for patients with hypertension and a history of myocardial infarction through remote monitoring and predictive analytics.

Supporting Type 1 diabetes self-management (UC3)

Improving treatment adherence, self-management and quality of care for children, adolescents and young adults living with Type 1 diabetes through personalised digital education and remote monitoring.

Personalised care for Type 2 diabetes (UC4)

Supporting more personalised treatment decisions and improved clinical outcomes for patients living with Type 2 diabetes and obesity through advanced imaging, AI-based risk prediction and patient-reported data.
